  • In-network insurances: Aetna, Regence, Anthem (Blue Cross Blue Shield), Moda, United Healthcare, UMR, Optum. I am NOT in-network with or take Medicaid/OHP or Medicare.

    • Initial Psychiatric Evaluation (90 minutes): $415

    • Follow Up Visits:

      • $200 (25 minutes)

      • $260 (45 minutes)

      • $315 (60 minutes)

    • Sliding scale for reduced rates available on a limited basis

    • Good Faith Estimates available upon request

Policies

  • Cancellation Policy: I require at least 24-hr notice for cancelling appointments. There is a late-cancel/no-show fee of $50.

  • A credit card is required to be on file prior to booking any appointments

  • Benzodiazepines such as Xanax (alprazolam), Ativan (lorazepam), Valium (diazepam), Klonipin (clonazepam), etc are rarely prescribed in this practice. If you’re seeking long-term use of benzodiazepines, I may not be the best match for you, but feel free to reach out to discuss further.
